August underground/Guinea Pig for graphic disturbance. Dont think it gets worse than that. But if like me, you like movies that work on a cerebral level too I would suggest anything by directors Gaspard Noe, Lars Von Trier, Michael Heneke, Harmony Korine, Todd Solonz, Patrice Chéreau etc etc. Check out the dogme 95 collective from Denmark.
The last movie I saw that fits the description of this thread would have to be Come & See.

We can say this about some Fellini movies or Bergman's, Ferreri, Kurosawa, Gherman, Sokurov, Abuladze, Paradjanov, just to name a few that were not mention before.
So every Real Filmmaker, author of cinema, is a bit twisted and disturbing. this is normal
